Supporting documents required for assistance

If you are applying for any assistance, please review the required documents needed for consideration. You can use our the "upload a document" service to submit a document, you may also email your documents to info@pinedalegov.org. 

We may have your CIB, Social Security Card, ID/Driver's License, and Homesite/residential lease on file already, please call and verify before submitting the required documents.

Covid-19 Assistance

COVID-19 Assistance (Emergency, Hardship and Utilities) Assistance is one household, one-time assistance only. Includes those who were assisted in 2020 to present.

1. Completed and signed Financial or Funeral Assistance Form

2. Copy of State issued ID or Driver's License

3. Copy of Certificate of Indian Blood

4. Copy of Social Security Card

5. If applying for emergency (Multi-purpose incident form, pictures, referrals, COVID-19 test result, other related documents)

6. If applying for hardship (referrals,, other related documents)

7. If applying for utilities (Utility bill in applicant name, referral, other related documents)

Financial or Funeral Assistance

Financial Assistance (Education, Emergency, Funeral, Hardship and Utilities)

1. Completed and signed Financial or Funeral Assistance Form

2. Copy of State issued ID or Driver's License

3. Copy of Certificate of Indian Blood

4. Copy of Social Security Card

5. If applying for education related expense (Letter, verification of school enrollment, other related documents)

6. If applying for emergency (Multi-purpose incident form, pictures, referrals, other related documents)

7. If applying for funeral expenses (Funeral expense cost form, other related documents)

8. If applying for hardship (referrals,, other related documents)

9. If applying for utilities (Utility bill in applicant name, referral, other related documents)

Housing Discretionary Assistance

Category A (No Homesite/Residential Lease)

1. Completed and signed Housing Discretionary Application

2. Copy of Applicant's and Co-applicant's Certificate of Indian Blood

3. Copy of all Household member's Social Security Cards

4. Certification of Income/No Income for all household members over the age of 18

5. Land Owner Authorization Form with copy of land owner homesite/residential or land documents

6. Referrals from Medical/Social/Other for any household members

7. Three (3) quotes from area vendors (Gallup Lumber, Bubany Supply, Lowe's, etc.)

8. If applicant and co-applicant have no income, no income tax - Certificate of No Federal Income Tax

Category B (Homesite/Residential Lease)

1. Completed and signed Housing Discretionary Application

2. Copy of Applicant's and Co-applicant's Certificate of Indian Blood

3. Copy of all Household member's Social Security Cards

4. Certification of Income/No Income for all household members over the age of 18

5. Copy of homesite/residential or land documents

6. Referrals from Medical/Social/Other for any household members

7. Three (3) quotes from area vendors (Gallup Lumber, Bubany Supply, Lowe's, etc.)

8. If applicant and co-applicant have no income, no income tax - Certificate of No Federal Income Tax

Category C (Bathroom Addition, Plumbing, electrical)

1. Completed and signed Housing Discretionary Application

2. Copy of Applicant's and Co-applicant's Certificate of Indian Blood

3. Copy of all Household member's Social Security Cards

4. Certification of Income/No Income for all household members over the age of 18

5. Copy of homesite/residential or land documents

6. Referrals from Medical/Social/Other for any household members

7. Three (3) quotes from area vendors

8. If applicant and co-applicant have no income, no income tax - Certificate of No Federal Income Tax

9. IHS/OEH Letter of Intent notice

Category E (Land Survey and/or Archeological Survey cost)

1. Completed and signed Housing Discretionary Application

2. Copy of Applicant's and Co-applicant's Certificate of Indian Blood

3. Copy of all Household member's Social Security Cards

4. Certification of Income/No Income for all household members over the age of 18

5. Copy of Homesite/Residential Lease application - Fish & Wildlife Biological Clearance form must be attached.

6. Referrals from Medical/Social/Other for any household members

7. Three (3) quotes from area vendors (Land Surveyor and Arch Clearance Surveyor)

8. If applicant and co-applicant have no income, no income tax - Certificate of No Federal Income Tax

1. Signed and completed Scholarship Financial Assistance Form

2. Copy of Certificate of Indianan Blood

3. Copy of Social Security Card

4. Copy of School ID or State 

5. Copy of letter of admission or enrollment verification

6. Copy of semester schedule
